When Should You Start Physiotherapy After a Hip Replacement?

Rehabilitation begins in hospital, usually on the day of surgery. You’ll be helped to stand, take your first steps with a walking frame, and practise basic movements like getting in and out of bed safely within your hip precautions.

Once you’re home, structured physiotherapy should ideally begin within the first week. The muscles around your hip respond best to targeted strengthening in the early post-operative period, and the movement patterns you establish in the first 6 weeks tend to stick , for better or worse. Early physiotherapy ensures those patterns are the right ones.

If you’re already several weeks or months post-surgery and feel your recovery has plateaued, it’s not too late. Preparing for Hip Replacement Surgery: Pre-Operative Physiotherapy

If your surgery is still ahead of you, starting physiotherapy before your operation , often called ‘prehab’ , can improve your post-surgical outcomes. Patients who go into surgery with better hip and core strength, improved balance, and practised confidence with walking aids typically recover faster and more completely.

Total Hip Replacement Recovery Timeline: Week by Week

Every patient’s recovery is different, but the following gives a general guide to the milestones we work towards:
Weeks 1–2
Early Mobilisation & Precaution Management The focus in the first two weeks is on managing pain and swelling, moving safely within your hip precautions, and beginning gentle strengthening exercises. We’ll ensure you’re confident with your walking aids, able to manage stairs safely, and moving around your home without unnecessary risk. Getting in and out of bed, dressing, and using the bathroom safely are key priorities.
Building Strength & Improving Your Walking Pattern This phase is where we focus on eliminating your limp. We progressively increase the intensity of your strengthening exercises , particularly targeting the gluteal muscles, which are critical to a normal walking pattern. We begin reducing your reliance on walking aids, typically moving from a frame to crutches to a single stick. Balance work becomes more prominent.
Functional Independence & Precaution Review Around the 6-week mark, many surgeons will review your precautions and may relax or remove them. This opens up your rehabilitation significantly. We work on longer walking distances, confidence on uneven ground, getting in and out of the car comfortably, and returning to hobbies and daily tasks. Most patients are cleared to drive an automatic car around 6 weeks.
Return to Full Activity For patients who want to return to more demanding activities , golf, swimming, cycling, gardening, travel , we continue to progress your programme beyond the 12-week mark. Hip replacement recovery is often quicker than knee replacement in terms of pain, but rebuilding full strength and eliminating a limp can take several months of consistent work.
