But the surgery itself is only half the story. The quality of your rehabilitation in the weeks and months that follow determines how much movement you regain, how quickly you recover, and whether you return to the level of function you’re hoping for.

In the first few weeks after knee replacement surgery, the last thing you need is the stress of travelling to a clinic. You’re often not recommended to drive for at least eight weeks, even as a passenger getting in and out of a car can be painful. Waiting rooms are uncomfortable. And the exercises you need to be doing are the ones that relate to your own home , your stairs, your chairs, your bathroom.

When Should You Start Physiotherapy After a Knee Replacement?

Rehabilitation begins in hospital, usually on the day of surgery or the day after. You’ll be helped to stand, take your first steps, and begin basic knee bending and straightening exercises before discharge.

Once you’re home, structured physiotherapy should ideally begin within the first week. This early window is critical , your knee is at its most responsive to treatment in the first 6 weeks post-surgery, and the range of motion you achieve during this period largely determines your long-term outcome.

If you’re already several weeks or months post-surgery and feel your recovery has stalled, it’s not too late. We regularly work with patients at 3, 6, or even 12+ months after their TKR who still have significant room for improvement. The sooner you start, the better , but starting late is always better than not starting at all!

Preparing for Knee Replacement Surgery: Pre-Operative Physiotherapy

If your surgery is still ahead of you, there’s strong evidence that starting physiotherapy before your operation , sometimes called ‘prehab’ , can improve your post-surgical outcomes. Patients who go into surgery with better muscular strength, range of motion, and confidence typically recover faster and more completely.

Total Knee Replacement Recovery Timeline: What to expect week by week

Every patient’s recovery is different, but the following gives a general guide to the milestones we work towards:
Weeks 1–2
Early Mobilisation & Pain Management The focus in the first two weeks is on managing pain and swelling, protecting the healing surgical wound, and achieving early range of motion goals. We’ll work on gentle knee bending and straightening exercises, get you moving safely and confidently with your walking aids, and ensure your pain medication is supporting your ability to participate in rehab. Most patients should be aiming for around 70–90 degrees of knee bend by the end of week two.
Building Strength & Reducing Dependence on walking aids This is where the hard work begins. We progressively increase the intensity of your strengthening exercises, work on normalising your walking pattern, and start reducing your reliance on crutches or a walking frame. Stair practice becomes a key focus. By week six, many patients are walking short distances without aids indoors and achieving 90–110 degrees of flexion.
Functional Independence The goal now shifts to getting you back to the activities that matter to you. We work on longer walking distances, confidence on uneven ground, getting in and out of the car, and returning to hobbies and daily tasks. Most patients are cleared to drive an automatic car around 6 weeks (manual may take longer , we’ll guide you on this). By 12 weeks, the majority of patients have returned to a good level of daily function.
Return to Full Activity For patients who want to return to more demanding activities , golf, swimming, gardening, travel , we continue to progress your programme beyond the 12-week mark. Some stiffness and swelling can persist for several months, and we’ll work with you to manage this while continuing to build your strength and endurance.

The NHS provides excellent surgical care, but post-operative physiotherapy provision varies significantly. Many patients are discharged from NHS physiotherapy after just a handful of sessions or a short group-based programme , often before they’ve reached the level of function they were hoping for.

If you’ve had both knees replaced (bilateral knee replacement) or are recovering from revision knee replacement surgery, your rehabilitation needs are very often more complex.
