A comprehensive home-based assessment is carried out within 5 working days of instruction. This includes a full physical evaluation, baseline outcome measures, home environment assessment, and discussion of the client’s goals and expectations.
A detailed report is delivered within 5 working days of the assessment, including clinical findings, baseline outcome measures, identified barriers to recovery, and a proposed treatment plan with goals, frequency, and estimated duration.
Treatment commences at the agreed frequency. Each session is documented and your client’s progress is tracked against their baseline measures and treatment goals.
Regular progress reports are provided at intervals agreed with you , typically every 4–6 weeks or at the end of a treatment block. Reports include updated outcome measures, progress against goals, and recommendations for ongoing treatment or discharge.
On completion of the treatment programme, a full discharge report is provided summarising the client’s journey, outcome measure comparisons from baseline to discharge, goals achieved, and any ongoing recommendations.
