Managing Long-Term Conditions With Physiotherapy

From Parkinson's and stroke to arthritis, COPD and heart conditions, regular physiotherapy helps you manage symptoms, stay active and protect your quality of life over the long term.

We build a sustainable programme of strength, mobility and endurance work, review it as your condition changes, and help you plan activity so you can do more with less fatigue and pain.

Many clients see us on a regular basis (weekly, fortnightly or monthly) to keep momentum, adjust exercises and catch small problems before they become big ones.

The pattern that works is little and often, sustained. We establish a baseline with proper outcome measures, build a programme you can genuinely keep up, and then review on a rhythm that suits you (weekly, fortnightly or monthly), adjusting as your condition, energy and priorities change.

Available across Southampton, Portsmouth, Fareham, Winchester, Salisbury and everywhere in between.

Who This Helps

People managing Parkinson's, multiple sclerosis, arthritis, COPD, heart conditions or general frailty: anyone for whom 'keep active' is the advice, but who wants a concrete, supervised plan for actually doing it.

Many clients see us on a standing schedule for years. The visits keep momentum, catch small problems before they grow, and give family confidence that someone expert is watching the trend.

Common questions

Long-Term Condition Management FAQs

Anything else? Ask us directly. A physiotherapist answers every enquiry.

Most clients settle at weekly, fortnightly or monthly: enough to progress the programme and spot changes early. It is always a shared decision and easy to adjust.

Appropriately dosed exercise is recommended in almost every long-term condition. Our job is the dosing: matched to your condition, your medication and how you are that day.

Yes. We regularly complement NHS reviews and specialist clinics, and with your permission we share notes so everyone is working from the same picture.

Honest ones: measured against your baseline at every review. Typically that means steadier walking and fewer bad weeks, plus clear conversation if something isn't working.

Arthritis, Parkinson's, multiple sclerosis, heart and lung conditions, osteoporosis, persistent pain and general frailty are the most common. If you live with something that slowly limits what you can do, an assessment will tell you honestly whether physiotherapy can help.

No referral is needed. With your permission we keep your GP informed, and if anything in our assessment needs medical review we will say so and help you arrange it.

Yes. Flare-ups and fluctuating energy are part of long-term conditions, so plans are built to scale up and down. On a rough week we adapt the session to what your body will comfortably allow.
