Rehabilitation After a Hospital Stay
The days after leaving hospital are critical. Confidence drops quickly, NHS community input can take time to start, and a short period of focused rehabilitation can be the difference between regaining independence and losing it.
We can usually visit within 48 hours of discharge, often sooner. Your first session focuses on the essentials, such as getting in and out of bed and chairs safely, managing the stairs and moving around the home, and builds from there.
Where useful we liaise with the hospital team, your GP and any care agency so that everyone is working to the same plan.
The first visit concentrates on the essentials: getting in and out of bed and chairs safely, managing the stairs, and moving confidently around your own rooms. From there the programme widens: longer walks, outdoor surfaces, and the specific tasks you want back, whether that is cooking a meal or getting to the paper shop.

How Post-Hospital Rehab Works
A typical programme after discharge looks like this:
- A first visit at home, usually within 48 hours of discharge
- Review of your discharge plan and any ward exercises
- Safety first: bed, chair and stair practice in your own home
- Visits weekly or more often while confidence rebuilds
- Liaison with your GP, ward team or care agency where useful
- A clear endpoint: goals met and a plan to stay strong

Anyone leaving hospital after surgery, infection, a fall or a long admission, especially people who live alone, or whose family are worried about how they will manage the first weeks at home.
Deconditioning after a hospital stay is fast but very reversible: the sooner structured rehabilitation starts, the more completely strength and independence return.

With their permission, we update family after every visit and flag anything that needs attention early. Many of our post-hospital clients are booked by a son or daughter living elsewhere.
Yes. We simply coordinate so the programmes complement each other, and we can hand over cleanly when NHS input begins or ends.
No. You can start privately as soon as you are home, and the first fortnight is when input matters most, while strength and confidence are at their lowest. If NHS community physiotherapy begins later, we coordinate around it.
Very little: your discharge summary if you have it, any walking aid the hospital gave you, and the exercise sheet from the ward. We assess everything else in the room it actually happens in, from the bed to the stairs.
Strength lost in hospital returns more slowly than it left, so expect steady weeks rather than instant results. Most people see meaningful change within a month of regular practice, and we measure progress at every visit so you can see it too.
